The PACE program provides all needed healthcare and support so older individuals can continue to live in their own home. Funded by Medicare and Medi-Cal, most people enrolled in PACE do not pay anything. PACE includes: 1. All healthcare 2. Emergency / hospital 3. Physical therapy 4. Dental, hearing, vision 5. Adult day health center 6. Meals 7. Recreational activities 8. Transportation 9. Prescriptions 10. Medical equipment 11. Home care 12. Mental health services 13. Rehabilitation 14. Caregiver support WHO IS ELIGIBLE? • 55 or older • Be certified as having long-term care needs • Live in a PACE service area • Can live safely in your community (with PACE support) • 99% are Medi-Cal eligible HOW MUCH DOES PACE COST? PACE is funded by Medicare and Medi-Cal. Most participants do not pay anything. There is never a co-pay, deductible, or coverage gap. However, if you have a Medi-Cal share of cost, you are still responsible for that cost. If a participant is not eligible for Medi-Cal, they can pay the Medi-Cal portion plus Medicare Part D. 99% of PACE participants are Medi-Cal eligible.
